Required Application Submittals (Standard and Checklist)

 1.  Quality Manual and/or Procedures Required by the Program.

1.1          All Programs—Welding Procedures

1.1.1       One sample welding procedure specification (WPS) for each processes used and procedure qualification record (PQR) (See AISC audit policy #2)

1.1.2       One welder qualification record (WQR) for each process used. (See AISC audit policy #2)

1.2          Building Standard

All procedures required and described by the Standard.

1.3          Conventional and Complex Building Structures

1.3.1       Bolt installation procedure

1.3.2       Inspection procedure considering receipt, in process and final inspection, who is responsible and what is recorded.

1.3.3       Non-conformance procedure

1.4          Major Bridge and Simple Bridge

1.4.1       Bolt installation procedure

1.4.2       Inspection procedure considering receipt, in process and final inspection, who is responsible and what is recorded.

1.4.3       Non-conformance procedure

1.4.4       Material Control Procedure

2.  Job/Position Descriptions

2.1          Descriptions must define reporting responsibilities, the position responsibilities and authorities related to the quality management system.

2.2          Include the qualifications required and describe the abilities necessary to successfully perform the functions assigned to the description.

3.  Biographical Information

3.1          Information about the personnel filling key quality management system positions.

3.2          Include the management of purchasing, detailing/engineering, fabrication processes, quality assurance, quality control and the key individual responsible for the quality management system (Management Representative.) Also include at a minimum: any individual whose position qualifications are specifically identified by the Program requirements.

3.3          The biographical data must show title(s) of the position held and that title must match a title on the organization chart or company organization description. Information must be included relating to the individual‘s qualifications for the responsibilities of the position(s) filled which may be illustrated by education, training, certifications, experience in the specific project types typically done by the company.

NOTE: An individual can fill more than one function in the quality management system.

4.  Equipment list

4.1          A list of the equipment used in fabrication and determined critical by the Fabricator to produce a quality produce in accordance with their quality management system.

4.2          The Fabricator must be able to define the basis for determining criticality. Depending on the basis, examples include: welding machines, punches, brakes, blasting equipment, paint equipment, mills, saws, drill lines, significant detailing equipment and stations.

5.  Facility plan

The facility plan shows the outline of major bays in the structural facility, the general location of major fixed equipment and cranes, and the relation of the building(s) to the property or lot. Include office and storage areas that address the required elements of the AISC Program quality management system. The plan does not need to be to scale or represent the legal definition of the facility.

6.  Organizational Description

6.1          An organization chart or other suitable means showing the positions that effect or relate to the quality management system, including higher levels of management and locations outside the facility.

6.2          At a minimum: the chart or description must include positions of the management of purchasing, detailing/engineering, fabrication processes, quality assurance, quality control and the key individual responsible for the quality management system (Management Representative or Certification Contact) Also include at a minimum: any individual whose position qualifications are specifically identified by the Program requirements, (for example: checkers, inspectors, welders.) For example, it is not necessary to show each worker, inspector, detailer, or purchasing agent separately, but supervision and management of these functions must be clearly shown. The chart or description may show position titles only, and not names. Position titles must correlate with position descriptions and/or biographical information. The chart or description must show formal reporting relationships and informal, informational (dotted line) relationships between positions and functions as they pertain to the quality management system. The description of the interactions and communication between the processes of the quality management system may be addressed in this chart/description or in combination with the required quality manual or procedures.
